The relevant Agreement will be electronically sent to you via Adobe e-signature service along with its accompanying UK GDPR & Data Protection Information Sheet.

These Agreements must be signed and returned before sessions can commence.
Menopause Wellness Coach-Client Agreement
Introduction
​​
We, the parties to this Agreement, hereinafter referred to as ‘Coach’ and ‘Client’, have agreed that Menopause Wellness Coaching services will be provided by the Coach to the Client in accordance with the following terms and conditions (*delete whichever not applicable):

Menopause Wellness Coach - Client Agreement
​
1. The Coach agrees to provide the Client with, and the Client agrees to pay for the agreed Menopause Wellness package/service (ticked above) that will be conducted on a weekly/fortnightly* basis (unless otherwise agreed).
​
2. Each Menopause Wellness session will run for a minimum of 45 minutes, unless ended earlier by the Client, or by mutual agreement.
​
3. Unless otherwise agreed with the Coach, the Client will pay (excluding VAT), by Bank Transfer. Payment is due before the block of 9 sessions, or at the commencement of each session. If the Client delays payment without the Coach’s agreement, the Coach will be entitled to refuse to provide further Menopause Wellness sessions to the Client. If the Client does not attend a session and provides less than 24 hours notice to the Coach to this effect, the Coach is entitled to charge the Client in full for that session.

4. The Coach agrees to conduct herself in terms of the New Insights Code of Ethics, to which the Coach is a signatory and which commits her to act fairly and in the best interests of the client at all times during coaching with the New Insights programme. The Coach will make a copy of the Code of Ethics available to the Client upon request.

5. The Client agrees that their primary motive for entering into this Menopause Wellness coaching agreement is for a supportive and safe space to help them navigate the perimenopause, menopause and post-menopause, with guidance and advice to create and sustain positive change in their life. For this purpose, the Client undertakes to work with the Coach in partnership, establishing and promoting an effective Menopause Wellness coaching and client relationship, by communicating openly with the Coach and making genuine and sincere efforts to do any work or action as set and agreed to during the Menopause Wellness sessions.

6. The Client acknowledges that the Menopause Wellness coaching service provided under this agreement is undertaken completely at the Client’s own risk. The Client understands that the coaching process may involve the Coach working with the Client to examine aspects of the Client’s life, entering into detailed discussion with the Client about the Client’s values and beliefs, making empowering requests of the Client and identifying and agreeing appropriate plans of action for the Client. The Client further acknowledges that, for coaching to be effective, the Coach may decide to ask the Client direct and personal questions and/or address issues, in the sessions that could cause the Client to experience strong emotions.
7. The Client acknowledges and agrees that the Coach does not guarantee results or provide warranties in respect of the Menopause Wellness coaching services and that the Coach shall not be responsible to the Client or to any third party for any physical or mental condition or circumstance caused as the direct or indirect result of, or related to, the Menopause Wellness coaching services in any way, including the Client’s failure to achieve agreed goals. The Client agrees to indemnify the Coach, Gateway Workshops™ (company that holds the Intellectual Property in the Practitioners Diploma in Menopause Wellness) in relation to any claim(s) made against them for loss or damage suffered as a result of the Client’s acts and omissions.
8. The Client acknowledges and agrees that the process of Menopause Wellness coaching does not constitute professional consultation, advice, counselling, therapy or medical treatment in any form. In the event that the Coach feels it appropriate for the Client to seek alternative professional assistance, the Coach may decide that Menopause Wellness coaching sessions should be discontinued or suspended for a period of time. In such cases the Client agrees to abide by the Coach’s decision and give due consideration to the Coach’s suggestions with regard to referral.
9. The Client agrees that the Client does not own intellectual property in, or have copyright over, any material provided to the Client by the Coach and that such material may only be used by the Client during coaching sessions for the purpose of being coached and may not be disclosed to any other person.

10. The parties agree to respect the confidentiality of any information provided to each other during the coaching sessions and not to disclose such to any third party without the other’s explicit consent. However, the coach reserves the right to break confidentiality under the following conditions:
​
a) Required by law (legally obliged disclosure) if she has been:

Told about an act of terrorism
-
Been made aware of drug trafficking and/or money laundering
-
Court orders or subpoenas
b) Permitted disclosures (where disclosure by the coach is discretionary):

Child protection and safeguarding
-
Protection of vulnerable adults
-
Risk of suicide or serious self-harm to client or harm to others

By initialling the box I declare that I understand, acknowledge and agree to the conditions
where the coach reserves the right to breach confidentiality.
11. The Coach is entitled to terminate this agreement if the Client fails to pay the Client’s coaching fees, otherwise breaches this agreement, has any insolvency proceedings brought against them, engages in harassment, abuse or discrimination against the Coach during sessions, is convicted of an offence or, in the Coach’s reasonable opinion, will not benefit from the coaching.
12. This document represents the Menopause Wellness coaching agreement between the parties and may only be varied in writing. This Contract sits alongside other provisions and policies for New Endeavours Life Coaching, including but not limited to the Privacy Policy and Data Protection Policy.

Life Coaching-Client Agreement

Life Coach - Client Agreement
​
1. The Coach agrees to provide the Client with, and the Client agrees to pay for the agreed coaching package/service (ticked above) that will be conducted on a weekly/fortnightly* basis (unless otherwise agreed).

For this purpose, the Client undertakes to cooperate with the Coach in establishing and promoting an effective coaching relationship by, amongst other things, listening carefully to the relevant information, tools and techniques provided to them by the Coach, communicating openly with the Coach and making genuine and sincere efforts to meet their goals as set and agreed to during the coaching sessions.

6. The Client acknowledges that all coaching services provided under this agreement are undertaken completely at the Client’s own risk. The Client understands that the coaching process may involve the Coach working with the Client to examine aspects of the Client’s life, entering into detailed discussion with the Client about the Client’s values and beliefs, making empowering requests of the Client and identifying and agreeing appropriate plans of action for the Client. The Client further acknowledges that, for coaching to be effective, the Coach may decide to ask the Client direct and personal questions and/or address issues, in the sessions that could cause the Client to experience strong emotions.

8. The Client acknowledges and agrees that the process of coaching does not constitute professional consultation, advice, counselling, therapy or medical treatment in any form.
